Quick answers to common questions about appointments, insurance, services, and more
Questions about booking, wait times, and what to expect
Most patients are seen within 24-48 hours of calling. For acute concerns like infections, injuries, or sudden illnesses, we often have same-day appointments available. If you're feeling very sick, call us as soon as you wake up and we'll do our best to fit you in that day. Research shows that timely access to primary care reduces emergency room visits and improves health outcomes.
Every appointment is scheduled for 30-60 minutes—far longer than the typical 7-10 minute visit at most practices. We believe in taking the time to truly understand your concerns, answer your questions, and create a comprehensive treatment plan. You won't feel rushed out the door. Studies in JAMA show that longer appointment times lead to better patient satisfaction, improved medication adherence, and better health outcomes.
Current patients can book appointments through our secure patient portal. New patients should call (908) 450-7002 for their first appointment so we can answer any questions and get you scheduled appropriately. After your first visit, you'll have online booking access for all future appointments at both our Bridgewater and Nutley locations.
Please call us at (908) 450-7002 as soon as possible if you need to cancel or reschedule. We require 24-hour notice for cancellations when possible, as this allows us to offer the appointment time to another patient who needs care. We understand that emergencies happen—just let us know and we'll work with you to find a new appointment time that fits your schedule.
We're open Saturdays 8am-2pm at both locations. Our Nutley office has extended hours on some weekdays until 7pm. For specific availability, please call (908) 450-7002. We don't currently offer Sunday appointments. We also offer telemedicine visits for many conditions, which can be scheduled during extended hours for added convenience.
Questions about coverage, costs, and payment options
We accept almost all major insurance plans including Medicare, Medicaid, Aetna, Blue Cross Blue Shield, Horizon BCBS, United Healthcare, Cigna, Humana, Oxford, and many others. Call our office at (908) 450-7002 to verify your specific plan is accepted. You can also visit our insurance and billing page for a complete list of accepted plans.
Yes, we accept Medicaid. We believe everyone deserves quality healthcare regardless of insurance type. Our team is experienced working with Medicaid and will help you navigate any coverage questions. We're committed to making healthcare accessible to all members of our community.
This depends on your insurance plan. Most patients with insurance pay their standard copay, typically between $10-50 for primary care visits. We'll verify your coverage before your appointment and let you know what to expect. If you don't have insurance, call us to discuss self-pay rates. We work with all patients to provide affordable care.
Most insurance plans and Medicare cover medical acupuncture at PremierMD—this is extremely rare in New Jersey. We'll verify your acupuncture coverage before your first treatment and let you know your copay or coinsurance amount. Even if not covered, our rates are reasonable compared to cash-only acupuncture practices. Learn more about our board-certified medical acupuncture services.
Most insurance plans cover Spravato for treatment-resistant depression. Our dedicated insurance team achieves a 95% approval rate. We'll verify your coverage and handle all prior authorization paperwork before you start treatment. Typical out-of-pocket costs are $10-40 per treatment session with insurance. Spravato is FDA-approved for treatment-resistant depression and major depressive disorder with suicidal ideation.
We accept cash, checks, credit cards (Visa, Mastercard, American Express, Discover), and HSA/FSA cards. Payment is expected at the time of service. We also offer payment plans for certain procedures or treatments—ask our billing team for details. You can manage payments conveniently through our secure patient portal.

Questions about what we offer and how we treat conditions
Family medicine doctors treat patients of all ages—from newborns to seniors. Internal medicine doctors (internists) only treat adults. At PremierMD, we provide family medicine, so we can be your doctor whether you're 5 or 95. Many families love having one doctor who sees everyone, from children to grandparents.
Yes! We provide comprehensive pediatric care from birth through age 18, including well-child visits, immunizations, sick visits, and management of chronic conditions like asthma or ADHD. We also treat adolescents and teens, making the transition to adult care seamless when the time comes.
Yes. Our providers (Dr. Boguslavsky and our nurse practitioners and physician assistants) are all licensed to prescribe medications, including controlled substances like ADHD medications, anxiety medications, and pain medications when medically appropriate. All prescriptions are sent electronically to your pharmacy for convenience.
We don't typically do house calls, but we offer telemedicine visits for patients who have difficulty coming to the office. Virtual visits work well for many conditions. For homebound patients with complex needs, we can sometimes arrange home visits—call (908) 450-7002 to discuss your situation.
Medical acupuncture is performed by a physician who has completed extensive training and board certification in the technique. Dr. Boguslavsky completed 300 hours of training at UCLA's Helms Medical Institute and is board certified in medical acupuncture. Unlike acupuncturists who only practice acupuncture, Dr. Boguslavsky combines his medical training with acupuncture expertise to create comprehensive treatment plans that may include medications, lifestyle changes, and other approaches.
Yes, we perform minor procedures including trigger point injections, joint injections, and wound care. We also do on-site lab work (blood draws). For procedures we don't perform, we coordinate with excellent specialists and ensure you get the care you need.
What to expect when you become a patient
Bring your photo ID (driver's license or passport), insurance card (front and back), a list of current medications (or bring the bottles), and any recent test results or medical records you have. Also bring questions you want to ask—write them down so you don't forget during the appointment! You can download new patient forms ahead of time to save time at check-in.
Plan for 45-60 minutes for your first appointment. This gives us time to review your complete medical history, discuss your concerns, perform a thorough physical exam, and create a treatment plan. We don't rush—bring all your questions. Learn more about what to expect at your first visit.
Yes! New patients can download forms from our patient forms page and complete them at home, or you can complete them online through our patient portal. This saves time at check-in. If you prefer to complete paperwork at the office, just arrive 15 minutes early.
You can request to see Dr. Boguslavsky or any of our other excellent providers (nurse practitioners and physician assistants). All our providers have access to your complete medical record and work as a collaborative team. Many patients see different providers for different needs—for example, Dr. Boguslavsky for acupuncture and Jackie (NP) for mental health care.
Most insurance plans don't require referrals to see a primary care doctor. However, some HMO plans do require referrals. Check with your insurance company or call our office at (908) 450-7002 and we'll help you determine if you need a referral. Visit our new patient information page for more details.
How to access your records and reach your doctor
After your first visit, we'll provide you with login credentials for our secure patient portal. You can access the portal from your computer, smartphone, or tablet to view lab results, schedule appointments, request prescription refills, and message your doctor. The portal is HIPAA-compliant and keeps your health information secure.
You can send secure messages through the patient portal for non-urgent questions. We respond within 24 hours on business days. For urgent medical concerns, please call the office at (908) 450-7002. For emergencies, call 911. The portal messaging system is HIPAA-compliant, unlike regular email which is not secure for health information.
Yes. Every patient receives Dr. Boguslavsky's personal cell phone number for urgent concerns outside office hours. This is for true urgent matters (sudden severe symptoms, medication side effects, unclear instructions). For non-urgent questions, use the patient portal and we'll respond the next business day. This level of accessibility is very rare in primary care practices.
Request refills through the patient portal or call our office at (908) 450-7002. Please allow 24-48 hours for refill requests to be processed. If you're out of medication, let us know it's urgent and we'll prioritize it. All prescriptions are sent electronically to your pharmacy for convenience.
Yes, we offer secure telemedicine visits via smartphone, tablet, or computer for many conditions. Telemedicine works well for follow-ups, medication refills, minor concerns, and when you can't take time off work to come to the office. Schedule telemedicine appointments the same way as in-person appointments. Research shows telemedicine is just as effective as in-person visits for many common conditions.
Questions about depression, anxiety, Spravato, and NJ patient certification programs
No referral needed. You can schedule directly with us for depression, anxiety, ADHD, or other mental health concerns. If you're already our patient for primary care, just mention at your next visit that you'd like to discuss mental health. We integrate mental health care into primary care for convenient, comprehensive treatment.
We can treat most depression, anxiety, and ADHD cases. Our providers are experienced with psychiatric medications and mental health treatment. We only refer to psychiatrists for very complex cases or when specialized psychiatric care is needed. Many patients prefer seeing their family doctor for mental health because we already know them and can coordinate all their care. Research shows primary care physicians successfully treat most common mental health conditions.
Spravato is an FDA-approved nasal spray medication for treatment-resistant depression (when you've tried at least two antidepressants without adequate relief) and for major depression with suicidal thoughts. Schedule a free benefits screening call at (908) 450-7002—we'll review your treatment history and insurance coverage to determine if you're a candidate. With over 20,000 treatments administered, we're New Jersey's most experienced Spravato provider. Research published in the New England Journal of Medicine shows Spravato's effectiveness for treatment-resistant depression.
Schedule a qualifying condition evaluation with Dr. Boguslavsky. He'll review your medical history and determine if you qualify under New Jersey law. If you qualify, he'll provide your certification immediately, and we'll guide you through registering with the NJ State Patient Registry Program (done online). The state typically approves registrations within 1-2 weeks. Unlike many certification services, Dr. Boguslavsky is a board-certified physician who can also provide ongoing primary care and monitor how state-authorized treatments affect your overall health.
Most insurance plans do not cover NJ patient registry evaluations or certifications. We charge a transparent fee for the evaluation. However, if you have an FSA or HSA account, these costs may be eligible for reimbursement. The state also charges a separate $20 registration fee. Call (908) 450-7002 for current evaluation pricing.
We prescribe anxiety medications when medically appropriate, including benzodiazepines like Xanax or Klonopin for short-term use. However, for chronic anxiety, we prefer medications that are safer for long-term use (like SSRIs or SNRIs) combined with therapy. We'll discuss the best options for your specific situation. We may also recommend acupuncture as a complementary treatment, as research shows it can be effective for anxiety management.
Managing diabetes, hypertension, and other chronic conditions
Typically every 3-6 months for stable chronic conditions, more frequently if we're adjusting medications or your conditions aren't well-controlled yet. We'll create a follow-up schedule based on your specific needs. Regular monitoring is important to prevent complications. Learn more about our chronic disease management approach, including specialized care for diabetes, hypertension, and thyroid disorders.
We can manage most chronic conditions including diabetes, hypertension, high cholesterol, thyroid disorders, asthma, COPD, and many others. For very complex cases or conditions requiring specialized procedures, we coordinate with excellent specialists while continuing to manage your overall care. You don't need five different doctors for five different conditions. Research shows that coordinated primary care management of multiple chronic conditions leads to better outcomes and lower healthcare costs.
Yes, we offer comprehensive weight management including nutritional counseling from our team of skilled providers, exercise guidance, and weight loss medications (including newer GLP-1 agonists like Ozempic, Wegovy, and Mounjaro) when appropriate. We use realistic, sustainable approaches—not fad diets. Clinical trials show GLP-1 medications can lead to 15-20% body weight loss when combined with lifestyle changes. We also offer acupuncture as a complementary approach that some patients find helpful for appetite control and metabolism support.
Sometimes, yes. If you make significant lifestyle changes (weight loss, diet improvement, regular exercise), some conditions may improve enough to reduce or eliminate certain medications. However, many chronic conditions require lifelong medication to prevent complications. We'll work with you to use the minimum medications necessary while keeping you healthy. Our integrative approach combines conventional medicine with acupuncture, nutritional counseling, and lifestyle medicine to optimize your health and potentially reduce medication burden over time.
